What type of referral would you like to make?

I am making a referral for

Hospice and end of life services Bereavement services Home care services

We advise using Google Chrome or Microsoft Edge when using the online referral form, instead of Internet Explorer.  

If you are a healthcare professional and you are unable to access the online referral forms using the buttons above, you can download the hospice and end of life services referral form and send it back to shh.singlepoint@nhs.net from a secure email address instead 

Make a referral

A referral can be made to St Helena by health and social care professionals, patients or their families.

Anyone over the age of 16 with complex problems related to an advanced, progressive, life-limiting or incurable illness can be referred to us for medical support. 

Adults who are struggling to cope with grief after the death of a loved one can also be referred to us for bereavement support. No prior connection to St Helena services is required to access our bereavement support.

Processing fees

When you make a donation to St Helena Hospice, we are charged transactional fees by other companies, including fees for processing payments made to us, looking up addresses and validating bank account details.

We are very grateful to our donors who offer to offset some of these fees with a minor addition to their total amount. This is however completely optional and we are very grateful for your support whether or not you choose to contribute to processing fees.
